Initial Consultation

At the initial visit, patients will be given a complete and thorough diagnosis, as well as a comprehensive explanation of treatment options available to them.  Please assist us by providing the following information at the time of your consultation:

• Your referral slip and any x-rays if applicable
• Completed new patient forms
• A list of medications you are presently taking

If you have dental insurance, bring your current insurance card. If you don’t have one, we will need the subscriber number, group number, and phone number. This will save time and allow us to help you in the processing of your claim.  Click here for more information on dental insurance.

IMPORTANT:  All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to treatment.  If your dentist or physician has taken x-rays, please have them forwarded to our office as they may be helpful in our diagnosis. A digital radiograph can be emailed to us. If additional films are necessary, they can be taken at our office.

If you are in extreme pain, we will do our best to see you, on most occasions, the same day that you call. We will make every effort to relieve your pain through emergency treatment, and then schedule you for the necessary follow-ups.  To schedule an appointment click here.
